Welp, I've never done this before. We'll see how it goes.
From to
Not hard, but probably more complicated than it needs to be. Made in CS3, non-translatable. PSD download
1) After some cropping and cloning, we start with this:

2) Add a texture (by innocent_lexys), and set to Overlay.
>>>
3) Not so fond of the flowers over Al's face there, so add a >vector mask, and use a soft brush to get rid of some of your texture. 25px 0% hardness worked nicely for me.

4) Another texture (by nineteen07); set to Color Dodge.
>>>
5) Aaaand another (by erniemay), set to Multiply, opacity 35%.
>>>
6) Add a selective color layer (Layer > New Adjustment Layer > Selective Color) with the following settings:
Reds Cyan: +72 Magenta: -42 Yellow: +9 Black: +68
Yellows Cyan: +56 Magenta: +5 Yellow: -1 Black: -30
Whites Cyan: +53 Magenta: -65 Yellow: -32 Black: +20
Neutrals Cyan: +34 Magenta: +23 Yellow: +9 Black: -17
Blacks Cyan: +37 Magenta: +10 Yellow: -20 Black: +18

7) Yet another texture (by nineteen07), Linear Burn, fill 9%.
>>>
8) And... another (by erniemay), Soft Light, fill 69%.
>>>
9) Add a curves adjustment layer (Layer > New Adjustment Layer > Curves), and get it looking something like this:

Lower left point: O 96, I 71 Middle point: O 141, I 138 Upper right point: O 166, I 160
You won't see a lot of difference, but work with me here.

10) Add another adjustment layer, this time levels. Set to 87, 1.76, 255, and you're done!
