Right, so I was recced a fic called Laundering Karma by RC Mclachlan.  The description I got was that it was B/J, post season 2 AU, Ethan and Justin broke up, and then there's a Columbine/Virginia Tech type shooting at PIFA. So I didn't have great expectations for the transplant fic, but it turned out to be really enjoyable, with one stellar chapter, good characterization of Brian and Justin in canon and an actual believable plot.
