Fan lacing help please!

Problem: When the twill tape is pulled to tighten the laces it bunches up to the top of the slides and the slides angle downward. I wish I had thought to take a photo on the client but this photo depicts pretty well what is happening. I have the laces passing through the last hole of the slide (inner row, 3rd down, it's  a5 hole slide) twice but it doesn't appear to be that that is the problem. ...
